The Family Centered Service Coordinator (FCSC) coordinates mental health (MH) services and supports for child welfare-involved families when two or more members of the family are experiencing—or are at risk of experiencing—serious mental illness or serious emotional disturbance. The FCSC focuses on the needs of the entire family and coordinates a holistic plan that addresses not only the individualized needs of specific family members but also the needs, goals and priorities of the family as a whole. Provide a full range of Family Centered Service Coordination services to a designated caseload of 10 to 12 families.
Work is performed with considerable degree of initiative and independent judgment in accordance with established regulations, policies, and procedures. Position reports directly to the FCSC Supervisor.
DUTIES AND RESPONSIBILITIES
- Implements and provides services according to the Single Point of Accountability responsibilities:
- Be the “go-to” resource for the client served and their family
- Assure that there are effective “safety net’ resources for the client served
- Clearly communicate to the client and their parent/guardian what they can expect from the system and what the system will expect of them
- Assure there is periodic assessment and cross system planning to meet the client’s needs while utilizing their strengths
- Prepare for, convene/facilitate service planning meetings and provide follow-up after meetings
- Assure there is cross system coordination of services and that services are being provided
- Provides culturally competent services with consideration for race, religious, sex, sexual orientation, age and ethnic background
- Assist the client in developing and utilizing natural supports
- Be a persistent advocate for those served and give feedback on systemic problems
- Provide a consistent positive outlook that encourages recovery and full inclusion in the community.
- Utilizes and promotes recovery principles in an effort to promote client’s resiliency and recovery.
- Maintains utilization targets quarterly that reflect daily activity and turns in case notes to the supervisor on a weekly basis. Document services in recovery-oriented manner.
- Provide a full range of Family Centered Service Coordination services to a designated caseload of 10 to 12 families.
- Maintain contact with each family on the caseload at least four times per month (once per week) during routine service coordination and at least twice per month for the six months following transition out of FCSC. Adjust the number of contacts according to the family’s need.
- Conduct timely assessments of client and family that thoroughly and accurately portrays strengths and
- needs. Assessments may include CANS, ANSA, North Carolina Family Assessment Scale (NCFAS), suicide risk assessment, violence risk assessment, etc.
- Promotes service planning efforts which result in developing, documenting and implementing a comprehensive service plan driven by the family utilizing all the agreed upon strengths and needs. Service plan will coincide with goals in the CYF Family Service Plan.
- Assure that there is effective “safety net” resources for the family through the development and reinforcement of Crisis Plans.
- Facilitates Interagency Team Meetings, including: being prepared for the meeting with necessary paperwork and background information, elicits all necessary information and brings focus to the meeting, identifies and explains their role within the team, clarifies responsibilities, and reinforces next steps at the close of the meeting.
- Provide ongoing monitoring of treatment plan by attending meetings and communicating with family and providers. As concerns arise, the service coordinator will provide feedback to provider as well as to the HSAO Supervisor.
- Monitor all treatment plans and any concerns with the contracted providers.
- Attend and participate in County Interagency Team Meetings
- Complies with and adheres to HIPAA confidentiality / Fraud, Waste, and Abuse policies.
- Coordinates with families, mental health providers, Children Youth and Families, juvenile probation, educational system, housing providers and any other community service providers as necessary to contribute to client’s success. Attend at all ancillary meetings with systems involved (court hearings, permanency planning, IEP, housing, etc.).
- Attends and participates in personal development and remains competent and knowledgeable of current trends, initiatives, resources and services available for clients and families.
- Provides culturally competent services with consideration for the family’s racial, religious, sex, SOGIE, age and ethnic background and identification.
- Enter family-related data into applicable spreadsheets and medical record systems.
- Other duties as assigned.
SKILLS AND KNOWLEDGE

ESSENTIAL REQUIREMENTS
- Possess Act 33, Act 34 and FBI clearance
- CPSL Mandated Reporter Training - Recognizing and Reporting Child Abuse
- Have a valid Pennsylvania motor vehicle license and access to a personal vehicle.
- Have current auto insurance that meets minimum collision coverage
- Knowledge of guidelines and regulations of all levels of Service Coordination Services.
- Strong oral and written communication skills.
- Excellent leadership qualities.
- Ability to work independently.
- Full-time position requiring 40.00 hours per week.

EDUCATION AND EXPERIENCE
(Preferred) Master’s Degree in Social Work, Education, Psychology, Criminal Justice, Counseling, Special Education, RN Degree, or related field and at least two years of work experience directly serving persons who have mental health needs and child welfare involvement; or
(Required) Bachelor Degree in Social Work, Education, Psychology, Criminal Justice, Counseling, Special Education with two years of work experience directly serving persons who have mental health needs and child welfare involvement. Strong interpersonal and organizational skills are necessary.
- Physical Demands. Requires vision, speech, and hearing and a moderate level of physical activity including; sitting, standing, walking lifting and carrying as required.
- Environmental Factors. Office setting for staff meeting, supervision and completion of paperwork, in-home community-based service delivery, some work from home may be allowed once utilization goals are met.
- Working Hours. Requires a minimum 40-hour workweek.
