HSAO’s Joint Planning Team program is an empirically based process that empowers Allegheny County’s most high-risk youth and their families by building a cohesive team of professionals and natural supports. This team utilizes the High-Fidelity model, including monthly team meetings, to design a personalized plan for each family that builds off their strengths in order to reach their vision for the future. JPT includes Coach, Facilitator, Family Support Partner, and Youth Support Partner.
The Supervisor will be responsible for day-to-day functions of the HSAO Joint Planning Team Program. HSAO’s JPT staff consists of a Coach, Facilitators and Youth Support Partners; the program collaborates closely with Allegheny Family Network’s Family Support Partners. The JPT Supervisor is responsible for ensuring the staff provide assessment, advocacy, linkage, and coordination of resources / services for the clients. Additionally, the supervisor along with the JPT coach will ensure that the program complies with the High Fidelity Wraparound model, principles, documents, phases, and Theory of Change as monitored by the Youth and Family Training Institute (YFTI). This position is responsible for compliance with utilization / documentation, reporting, quality assurance, etc. as outlined by HSAO, Allegheny County, CCBHO, and YFTI. The work is complex and involves interaction with consumers, their families, and providers.

Master’s Degree in Social Work, Psychology, Education, Counseling, Criminal Justice, Political Science, Sociology, Nursing, or related field
Three (3) years of experience providing services for adolescents and families
One (1) year that includes supervisory experience
Experience with Joint Planning Team / High Fidelity Wraparound.

Bachelor’s degree in Social Work, Psychology, Education, Counseling, Criminal Justice, Political Science, Sociology, Nursing, or related field
Five (5) years of experience providing services for adolescents and families
One (1) year of experience, that includes supervisory activities

DUTIES AND RESPONSIBILITIES
- Provide staff supervision
- Teach Facilitators and Youth Support Partners how to implement the High Fidelity Wraparound process with fidelity, including: modeling, observing meetings, reviewing documents, scoring skill sets, etc.
- Plan / Conduct weekly group coaching sessions for HSAO JPT Workforce
- Plan / Co-Facilitate quarterly coaching sessions for Allegheny County JPT Workforce
- Complete Youth and Family Training Institute Team Coach credential within 24 months and maintain credential throughout employment
- Prepare and review Professional Development Plans and performance evaluations by focusing on objective performance data
- Supervise staff planning and scheduling of day-to-day operations
- Utilize electronic health record and YFTI Dashboards to monitor quality, fidelity, and data analysis
- Ensure timely and accurate written documentation by staff
- Audit client records
- Monitor billable / utilization
- Report unusual incidents, trends, and problems to Unit Manager
- Resolve personnel issues along with the Unit Manager / Executive Director / HR
- Reporting to Allegheny County, CCBHO, YFTI, and other system partner as required
- Attend in-service and trainings required by Youth and Family Training Institute
- Participate as a member of the internal management team that links the activities of the various units and provides for quality assurance
- Maintain small active caseload on an as needed basis
- Accomplish goals identified in Leadership plan
- Interview perspective candidates
- Orientation and training of new staff
- Perform other duties as assigned.

- Possess thorough knowledge of county wide continuum of care services

- Have knowledge of the principles and practices of supervision
- Have the ability to exercise judgment in interpreting and implementing programs and procedures
- Have the ability to establish and maintain working relationships with families, staff, and community providers
- Have ability to communicate effectively both orally and in writing
- Ability to work in a team-oriented atmosphere
- Ability to be attentive to detail
- Demonstrated organizational / time management skills
- Ability to work with minimal supervision – to be self-directed in learning new skills
- Ability to work with people from a variety of diverse agencies, backgrounds, resources, and communities
ESSENTIAL REQUIREMENTS
- Possess Act 33, Act 34 and FBI clearances
- Have a valid Pennsylvania motor vehicle license and access to a personal vehicle
- Have current auto insurance that meets minimum collision coverage
- Knowledge of guidelines and regulations of Service Coordination Services
- Full-time position requiring 40 hours per week
- Ability to work independently

- Strong oral and written communication skills
