The Family Centered Service Coordinator (FCSC) coordinates mental health (MH) services and supports for families when two or more members of the family are experiencing—or are at risk of experiencing—serious mental illness or serious emotional disturbance.  The FCSC program gets referrals from the school districts and CYF.  The FCSC focuses on the needs of the entire family and coordinates a holistic plan that addresses not only the individualized needs of specific family members but also the needs, goals and priorities of the family as a whole. FCSC provides a full range of services to a designated caseload of 10 to 12 families.

Work is performed with considerable degree of initiative and independent judgment in accordance with established regulations, policies, and procedures.  Position reports directly to the FCSC Supervisor.

DUTIES AND RESPONSIBILITIES

  1. Implements and provides services according to the Single Point of Accountability responsibilities
  2. Utilizes and promotes recovery principles in an effort to promote client’s resiliency and recovery.
  3. Maintains utilization targets quarterly that reflect daily activity. Turn in case notes to the supervisor on a weekly basis. Document services in recovery oriented manner.
  4. Provide a full range of Family Centered Service Coordination services to a designated caseload of 10 to 12 families
  5. Maintain contact with each family on the caseload at least four times per month (once per week) during routine service coordination and at least twice per month for the six months following transition out of FCSC. Adjust the number of contacts according to the family’s need.
  6. Conduct timely assessments of client and family that thoroughly and accurately portrays strengths and needs.  Assessments may include CANS, ANSA, North Carolina Family Assessment Scale (NCFAS), suicide risk assessment, violence risk assessment, etc.
  7. Developing, documenting and implementing a comprehensive service plan driven by the family utilizing all the agreed upon strengths and needs.
  8. Assure that there is effective “safety net” resources for the family through the development and reinforcement of Crisis Plans
  9. Facilitates Interagency Team Meetings, including: preparing necessary paperwork / background information, establish meeting agenda, clarifies responsibilities, and reinforces action steps
  10. Provide ongoing monitoring of service plan by attending meetings and communicating with family and providers
  11. Monitor outside agency plans and address any concerns with providers
  12. Maintain positive, professional relationship with partner agencies, including CYF and assigned school districts
  13. Actively participate in County Interagency Team Meetings
  14. Complies with and adheres to HIPAA confidentiality / Fraud, Waste, and Abuse policies
  15. Coordinates with families, mental health providers, Children Youth and Families, juvenile probation, educational system, housing providers and any other community service providers to contribute to client’s success.  Attend at all ancillary meetings with systems involved (court hearings, permanency planning, IEP, housing, etc.).
  16. Attends and participates in personal development and remains competent and knowledgeable of current trends, initiatives, resources and services available for clients and families.
  17. Enter family-related data into applicable spreadsheets and medical record systems.
  18. Other duties as assigned.

ESSENTIAL REQUIREMENTS
 
  1. Possess Act 33, Act 34 and FBI clearance
  2. CPSL Mandated Reporter Training
  3. Valid Pennsylvania motor vehicle license and access to a personal vehicle
  4. Have current auto insurance that meets minimum collision coverage
  5. Knowledge of guidelines and regulations of all levels of Service Coordination Services
  6. Strong oral and written communication skills
  7. Excellent leadership qualities
  8. Ability to work independently
  9. Full-time position requiring 40 hours per week

Working Conditions
 
  1. Environmental Factors.  Office setting for staff meeting, supervision and completion of paperwork, in-home / community-based service delivery, some work from home may be allowed once utilization goals are met.
 
  1. Working Hours.  Requires a minimum 40 hour work week. 
Scheduled work hours are based upon the needs of the families and often require non-traditional hours, including early mornings and evenings. It is expected that the FCSC schedule home visits at times convenient for the families.

EDUCATION AND EXPERIENCE

(Preferred) Master’s Degree in Social Work, Education, Psychology, Criminal Justice, Counseling, Special Education or related field and at least two years of work experience directly serving persons who have mental health needs, special education services, or child welfare involvement

(Required) Bachelor Degree in Social Work, Education, Psychology, Criminal Justice, Counseling, Special Education, Nursing or related field and at least two years of work experience directly serving persons who have mental health needs, special education services, or child welfare involvement

Strong interpersonal and organizational skills are necessary.
