HSAO’s JPT program is an empirically based process that empowers the most high-risk youth and families by building a cohesive team of professionals and natural supports. This team uses monthly meetings to design a personalized plan for families to reach their vision for a better future. JPT includes a Facilitator, Family Support Partner, and Youth Support Partner.
The role of the Joint Planning Team (JPT) Coach is to support the development of the high-fidelity wraparound process by offering feedback to Facilitators and Youth Support Partners regarding strengths and areas of growth. The Coach will ensure direct service staff build competency to meet high-fidelity wraparound standards. Joint Planning Team Coach is responsible for modeling the process, partnering with staff to complete the JPT process, and providing ongoing consultation. The Coach will provide individual and group coaching, skills assessment, modeling, and documentation review to ensure fidelity and build knowledge and skills.
The JPT Coach maintains a partial caseload in order to pursue own JPT credential and model effective application of JPT Skill. The JPT Coach collaborates with JPT Supervisor on design and monitoring Youth Support Partner and Facilitator Professional Development Plans.
The work is complex and involves interaction with consumers, their families, Youth and Family Training Institute (YFTI), HSAO staff/supervisors, Family Support Partner Agency, and community service providers.


DUTIES AND RESPONSIBILITIES
 
  • Adhere to the 10 Principles of High-Fidelity Wraparound
  • Support client progress through the 4 Phases of High-Fidelity Wraparound
  • Provide coaching support to HSAO JPT Workforce, including observing meetings, reviewing documents, scoring skill sets
  • Complete Youth and Family Training Institute Team Coach credential within 18 months and maintain credential throughout employment
  • Plan / Facilitate individual and/or group weekly coaching sessions with HSAO JPT workforce
  • Plan / Co-Facilitate quarterly coaching sessions with Allegheny County JPT workforce
  • Utilize electronic health record / YFTI Dashboards to monitor timely completion of documents/surveys/chart forms and track program data
  • Report unusual incidents, trends, and problems to JPT Supervisor and Unit Manager
  • Orientation and training of new hire Facilitators/Service Coordinators and Youth Support Partners
  • Assist supervisor in arranging for staff development and training programs.
  • Attend in-service and training as required
  • Active participant in monthly YFTI Coaches calls
  • Maintain partial caseload
  • Meet utilization targets
  • Participate in related conferences, seminars, community outreach events
  • Accomplish goals identified in Professional Development Plans with JPT Supervisor
  • Perform other duties as assigned.

  • Demonstrated skills necessary to be a JPT Facilitator
  • Knowledge of culturally competent practices and the ability to demonstrate cultural humility
  • Possess thorough knowledge of countywide continuum of care and family serving systems
  • Have ability to plan, organize, and direct the work of others.
    • Have the ability to establish and maintain working relationships with families, staff, Youth and Family Training Institute (YFTI), HSAO staff/supervisors, Family Support Partner Agency, and community providers.
    • Have ability to communicate effectively both orally and in writing.
    • Ability to work in a team-oriented atmosphere.
    • Ability to be attentive to detail.
    • Demonstrated organizational skills.
    • Ability to work with minimal supervision – to be self-directed in learning new skills.
    • Ability to work with people from a variety of diverse agencies, backgrounds, resources, and communities.
ESSENTIAL REQUIREMENTS
 
  • Possess Act 33, Act 34 and FBI clearances
  • Have a valid Pennsylvania motor vehicle license and access to a personal vehicle.
  • Have current auto insurance that meets minimum collision coverage.
  • Strong oral and written communication skills.
  • Excellent leadership qualities.
  • Ability to work independently.
  • Full-time position requiring 38.75 hours per week.
EDUCATION AND EXPERIENCE
(Preferred) Experience with Joint Planning Team / High Fidelity Wraparound
and
 (Required) A bachelor’s degree in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, education or be a registered nurse and 5 years’ mental health direct care experience
