FUNCTIONAL DEFINITION


This individual will provide enhanced services to children / adolescents and their families. The facilitator will work with clients who have a substantial documented history of accessing community mental health services and are at high risk for hospitalization or out of home placement. This individual will be trained to implement the High Fidelity Wraparound Model by the Youth and Family Training Institute (YFTI). In Allegheny County, High Fidelity Wraparound is known as Joint Planning Team (JPT). The facilitator will utilize this model to provide outreach, assessment, advocacy, and linkage to the 12-15 identified clients on their caseload. The facilitator’s responsibility is to partner with the client and/or their parent/guardian to create a comprehensive, individualized service plan that will build upon the client’s strengths and support the client’s path to achieve their personalized Vision Statement. Collaboration will occur across the team, including client, family, natural supports, system partners -- as well as both youth and family support specialists. Work is performed with a considerable degree of initiative and independent judgment in accordance with established regulations, policies, and High Fidelity Wraparound Principles.  Position reports directly to the supervisor.



DUTIES AND RESPONSIBILITIES
 
  1. Complete High Fidelity Wraparound Facilitator Credential through Youth and Family Training Institute credential within 18 months and maintain credential throughout employment
  2. Maintains billable unit’s monthly reflecting daily activity and submits completed case notes within two business days
  3. Adhere to the 10 Principles of High Fidelity Wraparound
  4. Support client progress through the 4 Phases of High Fidelity Wraparound
  5. Maintain “fidelity” to the model as evidenced by both credentialing and re-credentialing
  6. Work collaborately with youth and family members of team, including Youth Support Partner and Family Support Partner.
  7. Attend any YFTI required meetings/coaching regarding model implementation and ongoing performance
  8. Effectively and efficiently manage a caseload as evidenced by maintaining contact with the family and the system partners monthly, facilitating interagency meetings, develop and monitor service plan goals, and oversee transition process to ensure that child and family receive all recommended services
  9. Collaborate with Children Youth and Families, Juvenile Probation, partner agencies, ACDHS, CCBHO, educational system and any other community resources. Attend all ancillary meetings (court hearings, permanency planning, IEP etc.) as indicated by service plan goals
  10. Complies with and adheres to confidentiality policy/HIPAA procedures as evidenced by keeping all files secure, obtaining appropriate consents, using HIPAA compliant communication protocols, and not releasing any third-party information
  11. Responsible for consumer electronic medical record chart to include all information as indicated by Designated Record Set
  12. Complete required surveys, chart forms, encounter forms as required by credentialing and funding entities
  13. Provide educational advocacy by attending any school meetings and providing technical assistance
  14. Monitor all service plans and address concerns with the contracted providers
  15. Attend and participate in personal development to remain competent and knowledgeable of current trends, initiatives, resources, and services available for clients and families
  16. Implement and provide services according to the Single Point of Accountability responsibilities:
    1. Be the “go-to” resource for the client served and their family
    2. Assure that there are effective “safety net’ resources for the client served
    3. Clearly communicate to the client and their parent/guardian what they can expect from the system and what the system will expect of them
    4. Assure there is periodic assessment and cross system planning to meet the client’s needs, while utilizing their strengths
    5. Prepare for, coordinate, and convene/facilitate service planning meetings and provide follow-up after meetings
    6. Assure there is cross-system coordination of services and that services are being provided
    7. Provide culturally competent services with consideration for race, religious, sex, sexual orientation, age and ethnic background
    8. Assist the client in developing and utilizing natural Be a persistent advocate for those served and give feedback on systemic problems
    9. Provide a consistent positive outlook that encourages recovery and full inclusion in the community
  17. Accomplish goals identified in Professional Development Plans with JPT Supervisor/Coach
  18. Other duties as assigned

ESSENTIAL REQUIREMENTS
  1. Possess Act 33, Act 34, and FBI clearances
  2. Have a valid Pennsylvania motor vehicle license and access to a personal vehicle
  3. Have current auto insurance that meets minimum collision coverage
  4. Knowledge of guidelines and regulations of all levels of Service Coordination Services
    1. Ability to communicate effectively both orally and in writing

    1. Full-time position requiring 38.75 hours per week



    Working Conditions
     
    1. Physical Demands.  Requires a moderate level of physical activity including sitting, standing, walking, stair climbing, lifting, and carrying as required.
     
    1. Environmental Factors.  Office setting for staff meeting, supervision and completion of paperwork; in-home / community-based service delivery. Some work from home may be allowed after six months once utilization goals are met and written approval is provided by supervisor.
     
    1. Working Hours.  Requires a minimum 38.75-hour work week. The position does not include weekend or on-call hours. Scheduled work hours are based upon the needs of the families and often require nontraditional hours, including early mornings and evenings. It is expected that the Facilitator schedule home visits at times convenient for the families.

    EDUCATION AND EXPERIENCE

    Bachelor’s degree with major course work in sociology, social work, psychology, human services, service coordination, gerontology, anthropology, political science, history, criminal justice, nursing, counseling, education, or related field and two years’ experience in a child serving system.  Strong interpersonal and organizational skills are a must.
